PCT & NURSE STUDENT PHYSICAL FORMS

The final date for your physical exam forms to be submitted is May 13, 2011.

If your physical form is incomplete when submitted on the final date, you will be ineligible to attend clinical with your class. I would advise everyone to submit their physicals for preliminary review *** no later than May 2, 2011****

Please begin to have your physical form completed NOW. If you wait and something is missing or not done properly you will not be able to attend clinical with your class.

This applies to PCT 110 students and NUR 100 as well as students who have completed PCT 110 and are making up other courses this term.

To avoid any conflict please submit your physical forms to Ms. Goias – Office between rooms 303 and 304.
